Accessing Medical Records from Sunrise Hospital
All protected health information (PHI) requests must be in writing. Please use Sunrise Hospital & Medical Center’s Authorization for Release of PHI to expedite your request for your medical records. Please forward all medical record request and a legible copy of your photo identification to:

Please note: Medical Records copied for reasons other than continuity of care are subject to a copy fee (NRS 629.061). These include but are not limited to legal requests, investigative agencies, insurance companies, and patient personal use requests. Birth Certificate Information
Certified copies of Birth Certificates for all babies born at Sunrise Children’s Hospital may be purchased from any Clark County Health District location.
